In order to help your first appointment run smoothly, please complete the New Patient Form below and bring it with you to your first appointment. Please note: the New Patient Form will open in a separate Internet Explorer window and is two pages in length. You may advance through the form by pressing the "tab" button on your keyboard. Where appropriate, you may click on items to place a checkmark on your answer. Having this form completed will help expedite your check-in process, however please arrive 15 minutes prior to your appointment time so the front desk can review your insurance benefits, our privacy policy and our financial policy.
Insurance Information
- BreakThrough Physical Therapy accepts Medicare, Workers Compensation, private pay, and most PPOs. For specific inquiries regarding your physical therapy benefits, please contact us at (408) 736-7600.
- In the state of California, physical therapists are able to treat as a primary provider on a private pay basis and with some insurance companies.
- As a courtesy, we verify insurance benefits for all patients prior to the first visit. However, we encourage each patient to contact their insurance company to verify the information for themselves, as the information we receive is not a guarantee.
What to Bring
- Physical therapy prescription
- Insurance card
- Loose, comfortable clothing (shorts, t-shirt, tennis shoes)
- Completed New Patient Form
- If the patient is under 18 years of age, a parent or legal guardian must be present during the first visit.
What to Expect
- Please arrive 20 minutes before your appointment to complete your paperwork (15 minutes if you’ve completed the intake forms online and brought them with you), and bring a change of clothes.
- Lockers and bathrooms are provided.
- Meet your therapist and spend approximately 40 minutes with him or her while he or she asks appropriate questions and performs appropriate tests to set up an effective and individualized treatment program based on your goals.
- Total appointment time is typically 1.25 hours to 1.5 hours. Individuals who require a more extensive gym program may spend more time than this at future sessions.
- If you have a limited amount of time for your visit, please inform your therapist at the beginning of the session so that he or she can ensure that your time is used most efficiently.
