Regardless of your situation, all of us at BreakThrough are committed to helping you achieve your goals. We are continually growing our word-of mouth (BiG) referrals based on this level of passion and we continue to help many achieve their goals as a result.
How it Works
Current Patients (currently in treatment with us):
- Let your physical therapist know of your goals
- Note: If we begin to see issues with your case, your therapist may bring it to your attention to get ahead of any surprises
- Your therapist will review your situation with our Accounts and Administrative Team
- Set a plan with your therapist for continued treatment via our Wellness Program
- Make any necessary account and scheduling modifications with our Client Service Team to convert to our Wellness Program
- Implement Wellness Program to completion or renew as appropriate
New or Returning Patients (new to BreakThrough or returning for any reason):
- Contact our Client Service Team at (408) 736-7600 or via our Contact Us Form
- Be prepared to discuss your situation and goals and provide needed contact information
- Our Client Service Team will review your case with appropriate personnel
- Our Client Service Team will contact you to clarify any unanswered questions and develop a personalized plan with you
- Note: If we will be billing “physical therapy” services rather than “wellness” services, a diagnosis from your medical doctor is currently required in California.
