Wellness Program

Whether you need and want continued care beyond what your insurance carrier deems as “medically necessary”, want to initiate a program to enhance your quality of life or prevent injury without involving your insurance carrier(s), BreakThrough Physical Therapy’s Wellness Program is the perfect solution.

“Wellness” has been described as “the constant, conscious pursuit of living life to its fullest potential”.  Regardless of your situation, all of us at BreakThrough are committed to helping you achieve your goals.  We are continually growing our word-of mouth (BiG) referrals based on this level of passion and we continue to help many achieve their goals as a result.

How it Works

 Current Patients (currently in treatment with us):

  • Let your physical therapist know of your goals
    • Note: If we begin to see issues with your case, your therapist may bring it to your attention to get ahead of any surprises
  • Your therapist will review your situation with our Accounts and Administrative Team
  • Set a plan with your therapist for continued treatment via our Wellness Program
  • Make any necessary account and scheduling modifications with our Client Service Team to convert to our Wellness Program
  • Implement Wellness Program to completion or renew as appropriate

 New or Returning Patients (new to BreakThrough or returning for any reason):

  • Contact our Client Service Team at (408) 736-7600 or via our Contact Us Form
  • Be prepared to discuss your situation and goals and provide needed contact information
  • Our Client Service Team will review your case with appropriate personnel
  • Our Client Service Team will contact you to clarify any unanswered questions and develop a personalized plan with you
    • Note:  If we will be billing “physical therapy” services rather than “wellness” services, a diagnosis from your medical doctor is currently required in California.

*Please note that Sports Performance and Wellness Services are not to be confused with physical therapy services and as such are not billable to most insurance carriers.
